Once you have completed that step fill out our Online Pet Profile. You will be contacted within 2 business days to setup an evaluation for your pooch.
Vaccinations records for Rabies, DHPP, and Bordatella must be received at or prior to the evaluation--no exceptions!

Once you have completed the form select the "print form" button on page 1 and bring the completed form, vaccination records and your furry companion to DDL for a quick evaluation.
